The terms and conditions of this Addendum will apply to all customers porting
into Celcom's network from existing network service provider ("Donor Operator").This Addendum shall be read together with the terms & conditions of Celcom Customer Registration Form; |

Celcom reserves the rights to change, amend and alter any part of this Addendum including the terms and conditions without prior notice to you and you hereby agree to be bound by such changes, amendments and alteration;

Mobile numbers that you wish to apply for porting must be within the range of mobile numbers approved by Malaysian Communication and Multimedia Communication (SKMM) subject however to the existing geographic numbering requirements; |
| 4. |
Celcom will process your application for mobile number porting between one (1) to five (5) days, subject however, to the availability of quota and approval by the Donor Operator. Should there be any delay by the approving Donor Operator or Celcom for any reason whatsoever, the processing period may exceed the time stipulated herein; |
| 5. |
You may experience service disruption when your existing mobile number is switched over from the Donor Operator to Celcom's network. Should you experience any service disruption, you are advised to call our Customer Careline at 1300 111000 or 1111 or e-mail to careline@celcom.com.my |
| 6. |
Porting from prepaid and post-paid mobile numbers to Celcom's prepaid and postpaid packages will be allowed subject to Celcom's existing terms and conditions; |
| 7. |
Any remaining credit in your prepaid mobile number will forfeited by the Donor Operator upon successful activation of your service by Celcom; |
| 8. |
All services inclusive of value added services (VAS) associated with your SIM/USIM and mobile numbers provided by the Donor Operator will be terminated upon successful activation of your service by Celcom; |
| 9. |
You shall be responsible for identifying supplementary lines or mobile numbers including but not limited to fax and data numbers as well that you wish to port along with the principal lines; |
| 10. |
You hereby agree and undertake to Celcom that you are the registered user of the mobile numbers and/or authorized principals for all the supplementary lines (if any); |
| 11. |
You may request to port in to Celcom's Network for and on behalf of a third
party provided you have submitted the following documents:
- An authorization letter signed by the registered user of the third party
mobile numbers;
- A photocopy of the third party national registration identity card / military ID / passport no. ; and
- A photocopy of your national registration identity card / military ID /
passport no.

| 12. |
Celcom shall not be responsible and/or liable for any and all losses, damages and/or misappropriation arising from this termination of your services with Donor Operator and you shall indemnify Celcom from any liability arising thereof; |
| 13. |
You shall be solely responsible for all charges incurred by Donor Operator, including but not limited to any and all outstanding payments and penalties charges due and imposed to you be the Donor Operator. In the event there is still non-payment of the outstanding charges owing by you to the Donor Operator, your service can be disrupted by us; |
| 14. |
Your mobile numbers request for porting shall be subject to approval from the Donor Operator and you will be liable for the new plans/lines subscribed with Celcom; |
| 15. |
You shall be required to sign a separate addendum and/or supplementary agreement for package and plans as requested by Celcom upon your submission of application for porting to Celcom; |
| 16. |
Celcom may impose a fee up to a maximum of Ringgit Malaysia Twenty Five (RM 25) for every number apply for porting upon registration or as stipulated in the addendum and/or supplementary agreement for the package and plan which you subscribed with Celcom and no refund shall be made for any rejection, cancellation, unsuccessful or withdrawal of the porting request due to any reason whatsoever; |

If the mobile numbers which you requested for porting, rejected by Donor Operator for any reason whatsoever, after settling the issue with Donor Operator, you can request re-submission of the porting request provided it is within thirty (30) days from the date of your application to Celcom and any processing charges will be at Celcom discretion; |
| 18. |
Without limitation to the fore-going , Celcom has the sole discretion to cancel the outstanding port request in the event your port request has been outstanding more than 21 (twenty one) business days from the day of the port request. |
